Purpose

  • Purpose of this position Provides clinical and administrative supervision for linkage clients into care.
  • Liaise with various partner organization, district health facility and government department.

Requirements
  • Grade 12 certificate
  • At least 2 years’ experience working as HAST Counsellor
  • HTS Certificate including Adherence Counselling and Rapid testing course
  • Advanced counselling certificate
  • Updated finger prick certificate
  • Clear criminal record
  • Proficiency in Microsoft Office
  • Ability to communicate in English and /Afrikaans or another South African language

Responsibilities
  • Strengthen linkages between the mobile HTS teams and the health facility
  • Community mobilization for HIV/AIDS prevention, care and treatment, PMTCT and TB/HIV services
  • Report weekly to the Professional Nurse on tracking and tracing of cases and sending referral list on the clients to the Bridge programme
  • Work closely with DOH to establish community based structures that will create demand for HIV/AIDS care, treatment.
  • Work closely with stakeholders and the role players on linking clients from mobile HTS teams into health and ART services to create demand for usage of mobile HTS teams in the community.
  • Coordinate activities with health facilities to ensure that clients referred from the mobile HTS receive the continuum of care within a health facility
