TB HIV Care (THC), in existence since 1929, is an internationally recognised non-profit organisation. We provide health care services throughout South Africa to both the general population and communities at increased risk for HIV and TB, including adolescent girls and young women, sex workers and people who use drugs. Our work culture is adapted to the constantly-evolving South African health care environment and will suit solution-driven team players interested in opportunities for career development while working to connect people with care Purpose of the position: To provide Home and Community Based Care programmes incorporating screening of non-communicable diseases, TB, HIV and STI’s including psycho-social needs within the sub-district

Responsibilities

Provides mentorship to Community Health Workers Coordinates and conducts awareness outreach in the community, workplace, tertiary educational facilities and schools in line with the health calendar. Appropriate referral of all cases identified in the community Ensures that clients referred to the Home and Community Based Care programme are managed Promotes service delivery and identifies training needs Networking and maintains a healthy and positive relationship with the sub-district Adheres to Infection control Standards Administration

Requirements

Grade 12 / matric Enrolled Nurse qualification 2 years related experience and/or training Registration with SANC as an Enrolled Nurse Recognized counselling certificate Proficiency in English and IsiXhosa languages Clear criminal record
